Аберрантные осцилляции и моторная дисфункция в модели болезни Паркинсона на крысах: новые терапевтические стратегии и цели

Болезнь Паркинсона (БП), прогрессирующее нейродегенеративное заболевание, обусловленное избирательной потерей дофаминергической иннервации базальных ганглиев (BG), что приводит к нарушению двигательной функции. При БП активность в нейронных моторных сетяx характеризуется появлением устойчивых 30-Гц бета-осцилляций, которые, как считается, нарушают обработку информации, связанной с движением, и приводят к возникновению моторной симптоматики (1-4). Леводопа устраняет эти осцилляции, что сопровождается временным восстановлением двигательной функции. Однако, при длительном лечении леводопой у большинства пациентов возникают серьезные побочные эффекты - леводопа-индуцированная дискинезия (ЛИД) c характерным появлением патологических 100-Гц гамма-осцилляций в таламусе и моторной коре (5-6). Механизмы, лежащие в основе патологической синхронизации в BG-таламо-кортикальных нейроцепях и их вклад в процесс возникновения двигательных нарушений и леводопа-индуцированной дискинезии, до конца не изучены. Bыявление критических компонентов нейросетей мозга, участвующих в этом процессе, может стать основой для разработки новых методов лечения пациентов с БП.

Мы исследовали эффекты обратимого торможения активности ядeр моторного таламуса (VM и Pf) и GPe (стратегическое ядро в BG; 7, 8) при локальном введении мусцимолa, агонистa ГАМК-A-рецепторов, а также повышения их активности пикротоксином, антагонистом ГАМК-A-рецепторов, на выраженность аберрантных ритмов и способности к восстановлению нормальной локомоции у крыс с экспериментальным паркинсонизмом. Нами показано, что при модуляции нейроннoй активности моторного таламуса и GPe (активация или торможение) мощность 30-Гц бета-осцилляций в BG-таламокортикальных цепях существенно уменьшалась, в то время как ее влияние на локомоцию не было однонаправленным. Отметим, что при торможении Pf таламуса и GPe двигательный дефицит устранялся, а при торможении VM таламуса моторный дефицит возрастал. И наоборот, повышение активности VM и GPe пикротоксином сопровождалось восстановлением нормальной локомоции, а введение пикротоксина в Pf усиливало двигательный дефицит. У крыс, получавших высокие дозы леводопы, приводящие к развитию ЛИД, введение мусцимолa в GPe и VM устраняло 100-Гц гамма-осцилляции в таламокортикальной нейросети. Однако, только торможение активности GPe предотвращало или в значительной степени снижало вероятность возникновения леводопа-индуцированной дискинезии.

Полученные результаты свидетельствуют, что наличие аберрантных осцилляций в моторных нейросетях не может полностью рассматриваться как первопричина двигательных нарушений при БП, но является свидетельством дисбаланса их активности. Важно, что GPe, как критический компонент BG, можно рассматривать в качестве главной мишени терапевтического воздействия с целью восстановления нормальной двигательной функции при БП и, возможно, предотвращении ЛИД. Отметим, что использование современных хемогенетическиx методoв позволит разработать новые подходы к устранению двигательных нарушений при болезни Паркинсона (9).

Aberrant oscillations and motor dysfunction in the rat model of Pakinson's disease: new therapeutic strategies and goals

Parkinson's disease (PD) - a progressive neurodegenerative disease caused by a selective loss of dopaminergic innervation of the basal ganglia (BG), resulting in impaired motor function. In PD, activity in neural motor networks is characterized by the appearance of sustained 30-Hz beta-oscillations, which are thought to impair movement processing and lead to motor symptoms (1-4). Levodopa eliminates these oscillations, which is accompanied by a temporary restoration of motor function. However, with long-term treatment with levodopa, most patients experience serious side effects - levodopa-induced dyskinesia (LID) with the characteristic appearance of pathological 100-Hz gamma-oscillations in the thalamus and motor cortex (5-6). The mechanisms underlying pathological synchronization in BG-thalamocortical neurocircuits and their contribution to the process of occurrence of the PD motor symptoms and levodopa-induced dyskinesia are not fully understood. Identification of the critical components of the brain neural networks involved in this process can become the basis for the development of new methods of treating patients with PD.

We investigated the effects of reversible suppression of the activity of the nuclei of the motor thalamus (VM and Pf) and GPe (the strategic nucleus in BG; 7, 8) by local administration of muscimol, a GABA-A receptor agonist, or increasing their activity with picrotoxin, a GABA-A-receptors antagonist, on the severity of aberrant rhythms and the ability to restore normal locomotion in rats with experimental parkinsonism. We have shown that, upon modulation of the neuronal activity of the motor thalamus and GPe (activation or inhibition), the power of 30-Hz beta-oscillations in the BG-thalamocortical circuits significantly decreased, while its effect on locomotion was not unidirectional. It should be noted that upon inhibition the activity of thalamic Pf and GPe, the motor deficit was eliminated, while after inhibition of thalamic VM the motor deficit increased. Conversely, an increase in the activity of VM and GPe with picrotoxin was accompanied by the restoration of normal locomotion, but the introduction of picrotoxin into Pf increased the motor deficit. In rats treated with high doses of levodopa, leading to the development of LID, infusion of muscimol into GPe and VM abolished 100-Hz gamma-oscillations in the thalamocortical neural network. However, only inhibition of GPe activity prevented or significantly reduced the incidence of levodopa-induced dyskinesia.

The obtained results indicate that the presence of aberrant oscillations in motor neurocircuits can not be fully considered as the cause of motor symptoms in PD, but to some extend this could be the manifestation of the imbalance in brain activity. It is important that GPe, as a critical component of BG, can be considered as the main target for therapeutic intervention in order to normalize motor activity in PD and, possibly, prevent LID. Note that the use of modern chemogenetic methods will allow the development of new approaches to the elimination of movement disorders in Parkinson's disease (9).
