Биохимическая in vitro диагностика и направленная доставка лекарственных препаратов in vivo на основе магнитных наночастиц

Магнитные наночастицы (МЧ) из оксидов железа, обладая рядом уникальных свойств, перспективны для многих биофизических и медицинских применений. Железо в разных формах присутствует в организме человека в количестве около 5 г, обеспечивая жизненно важные функции. Нанооксиды железа обладают низкой токсичностью и допущены во многих странах для внутривенных инъекций человеку.

Для метрологического обеспечения биофизических разработок авторами впервые был предложен принцип высокочувствительной количественной регистрации МЧ в непрозрачных 3D объектах по нелинейному перемагничиванию. На основе этого принципа и синтезированных МЧ разработаны новые способы:

1) неинвазивной «жикостной биопсии», экспрессной биохимической диагностики и контроля безопасности пищевых продуктов;

2) эффективной доставки лекарственных препаратов к раковым опухолям и метастазам в организме лабораторных животных;

3) комплексного исследования долговременной биодеградации МЧ и их токсичности in vivo, а также выявления продуктов распада наночастиц в живом организме.

С помощью применения МЧ в качестве меток иммуно- и ДНК-реакций разработан ряд экспрессных методов измерений в биологических жидкостях человека концентраций малых молекул, белковых маркеров заболеваний, а также внеклеточных везикул (ВВ). ВВ, являясь носителями белковых и ДНК фрагментов раковых клеток, перспективны для неинвазивной «жидкостной биопсии» [1]. Предел обнаружения ВВ в клинических образцах пациентов с раком молочной железы составил 3,7·10e5 шт/мкл, что на 1-2 порядка лучше, чем у наиболее чувствительных коммерческих тестов. Достигнут предел обнаружения простат-специфического антигена (ПСА) в сыворотке крови 19 пг/мл при динамическом диапазоне, превышающем 3,5 порядка концентрации. Метод перспективен для выявления как самого рака простаты, так и его рецидивов после радикальной простатэктомии по следовым количествам ПСА [2]. Предел обнаружения сердечного тропонина I составил 0,08 нг/мл при динамическом диапазоне более 3 порядков, что важно для ранней диагностики инфаркта миокарда [3]. Разработан метод для быстрого (20 минут) измерения концентрации малых молекул, в частности охратоксина А, в продуктах питания от 11 пг/мл при динамическом диапазоне 5 порядков, что находится на уровне эталонных, но трудоемких и длительных способов [4].

Применительно к терапевтическим задачам заметим, что большинство наноносителей препаратов становятся малоэффективными in vivo из-за быстрого их выведения из кровотока системой мононуклеарных фагоцитов (МФС). Нами разработана прорывная технология в наномедицине, которая позволяет значительно (в 32 раза) увеличить время циркуляции в крови практически любых нанопрепаратов, повышая их терапевтическую эффективность [5]. Установлено, что применение этой технологии, названной «МФС-цитоблокадой», приводит к существенному увеличению доставки химиопрепаратов к пяти видам опухолей различной природы: от меланомы до рака молочной железы, включая два типа опухолей человека, привитых мышам. В частности, показано, что при использовании технологии в 23 раза возрастает эффективность “активной” магнитной доставки химиопрепаратов в раковые опухоли у животных. Это приводит к значительному подавлению опухолей при минимизации побочных эффектов. Впервые продемонстрировано, что нацеливание полимерных и магнитных наночастиц лектинами к профилю гликозилирования раковых клеток с последующей фотодинамической терапией является многообещающей стратегией лечения агрессивных опухолей [6].

Важным аспектом при создании агентов для адресной доставки препаратов является их быстрая биодеградация после выполнения терапевтических функций. В экспериментах с животными in vivo впервые проведено всестороннее исследование полного жизненного цикла МЧ, начиная с введения в кровоток и вплоть до биодеградации в живом организме [7,8]. Установлены закономерности долговременной (1 год) биодеградации для 17 типов наночастиц в зависимости от введенной дозы, гидродинамического размера, ζ-потенциала, типа покрытия поверхности и структуры наночастиц. В частности, показано, что покрытие частиц 39-нм слоем полистирола замедляет время деградации МЧ с 40 дней до 1 года. Установлена динамика снижения контрастирующих свойств МЧ для МРТ и регистрации по нелинейному перемагничиванию. Обнаружено, что биодеградация МЧ приводит к усилению экспрессии генов железо-ассоциированных белков, увеличению количества эритроцитов и уровня гемоглобина в крови животных, а также отсутствию значимой токсичности МЧ. Полученные результаты важны для разработки и внедрения разных типов наночастиц в клиническую практику.

Biochemical in vitro diagnostics and targeted drug delivery in vivo based on magnetic nanoparticles

Magnetic nanoparticles (MPs) based on iron oxides possess a wide range of unique properties and are promising for many biophysical and medical applications. 5 g of iron is present in the human body in various forms and are vital for organism’s functioning. Iron nanooxides feature low toxicity and are approved in many countries for intravenous injecting to humans.

As a metrological tool of developments in biophysics, the authors introduced a principle of highly sensitive quantification of MPs in opaque 3D objects using nonlinear remagnetization. Based on the principle and synthesized MPs, new methods have been developed for:

1) non-invasive "liquid biopsy", express biochemical diagnostics and food safety control;

2) effective delivery of drugs to cancer tumors and metastases in the animals;

3) a comprehensive study of the long-term biodegradation of MPs and their toxicity in vivo, as well as the identification of degradation products in a living organism.

Based on the use of MPs as labels for immune and DNA reactions, a variety of express methods were developed for measuring the concentrations of small molecules, protein markers of diseases, and extracellular vesicles (EVs) in human biological fluids. The EVs, being carriers of protein and DNA fragments of cancer cells, are promising for non-invasive “liquid biopsy” [1]. The limit of EV detection in clinical samples of patients with breast cancer was 3.7·10e5 pcs/µl. That is 1-2 orders of magnitude better than that of the most sensitive commercial tests. The limit of detection of prostate-specific antigen (PSA) in blood serum of 19 pg/ml was obtained at a dynamic range > 3.5 orders. That is promising for diagnosis of both prostate cancer and its relapses after radical prostatectomy using trace PSA amounts [2]. The limit for cardiac troponin I detection was 0.08 ng/ml at a dynamic range > 3 orders, which is important for early diagnosis of myocardial infarction [3]. Another method enables rapid (20 min) measuring of small molecules concentrations, in particular ochratoxin A in foods from 11 pg/ml within a dynamic range of 5 orders. That is on the level of reference methods, which are labor- and time-consuming [4].

With regard to therapeutic tasks, we note that most drug nanocarriers become ineffective in vivo due to rapid elimination from the bloodstream by the mononuclear phagocytes (MPS). We have developed a breakthrough technology in nanomedicine, which enables significant (32-fold) increase of circulation time in the blood of almost any nanodrugs. That increases their therapeutic efficacy [5]. It was demonstrated that this technology called "MPS-cytoblockade", led to a significant increase in the delivery of chemotherapy drugs to five types of tumors of various nature: from melanoma to breast cancer, including two types of human tumors inoculated into mice. In particular, it was shown that it increased the efficiency of “active” magnetic delivery of chemotherapy drugs to cancerous tumors in animals by a factor of 23. This produced a significant suppression of tumors while minimizing side effects. Besides, it was shown that targeting polymeric and magnetic nanoparticles with lectins to the glycosylation profile of cancer cells, followed by a photodynamic therapy, represents a promising strategy for the treatment of aggressive tumors [6].

An important aspect for creating drug delivery agents is their rapid biodegradation after performing therapeutic tasks. In the experiments with animals in vivo, a comprehensive study of the complete life cycle of MPs was carried out for the first time, starting from their introduction into the bloodstream of animals and up to biodegradation in a living organism [7, 8]. Dependences were established for long-term (1 year) biodegradation of 17 types of nanoparticles upon injected dose, hydrodynamic size, ζ-potential, type of surface coating and structure of nanoparticles. In particular, it has been shown that coating the particles by a 39-nm layer of polystyrene prolongates the MP degradation from 40 days to 1 year. The dynamics of decay of the contrasting properties for MRI, as well as for detection by non-linear remagnetization have been established. It has been found that MP biodegradation enhances expression of genes of iron-associated proteins, increases the number of erythrocytes and the level of hemoglobin in the blood, as well as the absence of significant toxicity of the MPs. The results are important for employment of different types of nanoparticles in clinical practice.
